Deactivate vs Remove:
- Deactivate - when a clinic user is no longer practicing
- Remove - when a clinic user is no longer working with a specific virtual clinic
This must be requested through your HSP and eHealth by completing and submitting one of the forms listed below. The user being removed or deactivated will not be notified.
Step 1 - Complete a form to request removal from a clinic and/or to deactivate your account:
Account request/deactivate form used by SHA Users –
- Request a 'new' user account
- Update user account info (name, email, mobile # or job title)
- Add an active user to a virtual clinic
- Remove a user from a virtual clinic
- Other change requests (i.e. account deactivation)

- Request a 'new' user account
- Remove a user from a virtual clinic
- Change requests (i.e. account deactivation, update name, email, mobile # or job title)

Step 2 - Please complete the following before requesting one of the above changes to a healthcare provider's account:
- Notify your patients that their healthcare provider is leaving
- Cancel and reschedule all upcoming appointments patients may have with the healthcare provider - patient consultations or 'Meet Now' meetings
- Archive any active patient consultations
NOTE: There is currently no way to transfer a patient to another clinic. If a healthcare provider is moving to a new clinic, patients will have to be invited to the new clinic. They will remain a patient of the current clinic and their data will remain in the current clinic.
