- Gender
- Address
- Phone Number
- Health Card Number
Under 'Pre-Consultation Checklist' in the middle of your screen, complete each section.

After completing the 'Pre-Consultation Checklist', go to your 'Profile' under 'Account' and complete the necessary information there. Remember to 'Save Changes' whenever you add or revise information in your Profile.
