- Date of Birth (mandatory)
- Gender
- Address
- Phone Number
- Health Card Number
Under Pre-Consultation Checklist in the middle of your screen, complete each section.

After completing the Pre-Consultation Checklist, go to your Profile under Account and complete the necessary information there. Remember to Save Changes whenever you add or revise information in your Profile.
