This must be requested through your HSP or eHealth by completing and submitting one of the forms listed below. The user being removed or deactivated will not be notified.
Step 1 - Complete a form to request removal from a clinic and/or to deactivate your account:

Step 2 - Please complete the following before requesting one of the above changes to a healthcare provider's account:
- Notify your patients that their healthcare provider is leaving
- Cancel any upcoming appointments patients may have with the healthcare provider - patient consultations or 'Meet Now' meetings
- Archive any active appointments patients may have
NOTE: There is currently no way to transfer a patient to another clinic. If a healthcare provider is moving to a new clinic, patients will have to be invited to the new clinic. They will remain a patient of the current clinic and their data will remain in the current clinic.
