Step 1 - Raise a request for your account deactivation by sending an email:

Step 2 - Provide your name, email address, and indicate that you wish to deactivate your account. If possible, please describe the reason for the account deactivation.
- You may withdraw your consent to provide personal health information (PHI) and personal information (PI) at any given time, however this withdraw is not retroactive.
- Please be aware that deactivating your account will restrict your ability to log in and see your past information.
- However, even after account deactivation your health information will be stored and kept safe for the retention period according to the provincial legislations (Health Information Privacy Act).
